A 61-year-old woman is dead after a crash involving five vehicles on Interstate 295 south of Old St. Augustine Road, according to Florida Highway Patrol.

FHP said the second car slowed for traffic, and the 61-year-old woman failed to slow down and rear-ended the car in front of her. The woman attempted to move to the center lane and pulled in front of a third car that was trying to avoid the initial crash, according to FHP.The woman was then redirected into the HOV lanes. Investigators said she traveled directly in front of a fourth vehicle in the outside HOV lane where the front of the fourth car collided with the driver’s side of the woman, which caused her car to rotate.The fourth car came to a final rest and blocked the inside travel lane and outside HOV lane of I-295.
