Barnsley Council apologises after more than 1,000 votes went uncounted in last May's election.
Sarah Norman, chief executive of Barnsley Council, said just days after the vote the error was "unacceptable"
The Association of Electoral Administrators carried out an independent investigation and concluded there was no evidence of actual or attempted electoral fraud and that the incident was simply "human error". However, they were "satisfied that the officer has taken steps to make sure that our count processes are designed and managed to secure an accurate result in the future".
