Emergency services raced to the A93, near Meikleour, after the alarm was raised at around 11.40am on Wednesday.
The crash involved the Mercedes car towing a caravan, a Ford Transit Connect van and a Fendt tractor and trailer.
The road remains closed to allow a collision investigation to be carried out and police are now appealing for information.
