A man on remand at Forest Bank prison told his sister other prisoners there bullied him and threatened to sexual assault him, a newly-published report reveals. And she alleged staff at the prison in Salford took no action over his claims and didn't check on his welfare or offer him any support.

Staff at Risely are said to have 'recorded little' about him in the 10 days leading up to his death on October 26, 2022. He was pronounced dead in his cell despite resuscitation efforts.
His sister, says the report, made eight calls to Forest Bank over two days in the August before he was transferred. "She said that her brother had told her he was going to kill himself due to abuse from other prisoners," said ombudsman Adrian Usher in the report, which also revealed Ballard told officers he believed he had 'a price on his head'.
