A difficult and distressing day of evidence heard of errors and mix-ups in the care provided to Dan Kay before he died
There were 'huge mistakes', communication issues and errors made by mental health teams in the days leading up to the death of a hugely popular and respected Liverpool journalist, an inquest has heard. Former Liverpool ECHO journalist and tireless Hillsborough campaigner Dan Kay died on Sunday May 7 2023 at the age of 45 after being hit by a train near to Mossley Hill Train station.
He added: 'Risk is always there but I felt the positive nature of the visit meant we could move to a telephone call. He was happy to do that with people he had met before.' The coroner pointed out that this change to the visits was not recorded in Dan's care plan, to which Mr Potter said: 'I should have updated that. That is a fault of mine.
