]. Differentiating COVID-19 and influenza by symptoms is therefore difficult, and testing is needed to accurately diagnose the causative pathogen in children with acute respiratory illness when both viruses cocirculate.
Our comparative evaluation had notable strengths and limitations. Both the influenza and COVID-19 cohorts were multicenter, geographically diverse, and limited to children admitted to the ICU, allowing comparison of acute severe respiratory illnesses. In conclusion, despite notable differences in the demographic and clinical characteristics of unvaccinated children with COVID-19 in the pre-Delta and pre-Omicron variant eras compared with children with influenza admitted to the ICU, both viruses were associated with critical illness and the need for life-saving measures.
