A 51-year-old man, Paul Skinner, was found dead on a public bench a day after being released from Forest Bank Prison. The report reveals the cause of death as multiple drug toxicity and alcohol poisoning. Despite efforts to support him with community services and housing, he was unable to maintain his sobriety and became homeless.

A man was found dead on a public bench just a day after his release from Forest Bank Prison. Paul Skinner was 51. He was released from the prison in Salford on February 20 this year, but was found 'unresponsive and not breathing' on a 'public bench' by two homelessness outreach workers the following day, a report into his death has revealed.

Police and paramedics attended the scene, but he was pronounced dead. A post-mortem examination found Skinner died from 'multiple drug toxicity', the published report by the Prisons and Probation Ombudsman reveals. A toxicology report, meanwhile, identified 'evidence of potentially fatal alcohol poisoning' and that he used methadone and cocaine. A verdict 'of alcohol and drugs' was recorded by a coroner after an inquest into his death. The report said an investigation found that his community offender manager 'liaised with several different agencies to ensure that his needs were met'. 'She told the investigator that this became increasingly difficult because his behaviour meant that some of the supported housing providers were not willing to offer him another place,' said the ombudsman Adrian Usher in his report. 'While in prison, Mr Skinner underwent detoxification and he was supported by the prison's substance misuse services. 'When leaving prison, appropriate arrangements were made with community services to support him with his abstinence. This included referrals to abstinence supported housing and linking him with community support groups. Sadly, Mr Skinner was not able to maintain his sobriety, which resulted in him becoming homeless.' Skinner, who was sent to HMP Forest Bank on February 11 this year after spells in other prisons and recalls to custody after releases, was described in the report as a 'persistent offender' with a history of alcohol and drug use. 'As a result, he frequently returned to prison, which disrupted any progress he made during his release,' Mr Usher added. 'His short stays in prison prevented him from benefiting fully from the available interventions.' A substance misuse worker met with him before his release on the day and an appointment was made for him to see a community services team later the same day. But the report said his community offender manager couldn't trace him after his release and he didn't turn up for an appointment with the probation service. He was found dead the next day.
