A mum has spoken of her heartbreak after her son was found dead inside his cell at a Salford prison after being prescribed a 'combination' of drugs.
She told the court she 'could not understand' how her son died whilst in the care of 'those supposed to be taking care of him.' A toxicology report found mirtazapine, pregabalin, olanzapine, and methadone - all of which he had been prescribed - present in his blood after he died. Doctors also found quetiapine, which he was not prescribed, but said they were unable to identify any single drug as being responsible for his death.
Lindsey Ward, who conducted the toxicology report, said the level of methadone in Mr McDonagh's blood was within a usual therapeutic range but it 'potentially' could have been 'the straw that broke the camel's back' depending on his level of tolerance to methadone and other opioids.
