Nottingham City Council has confirmed details of an independent review into the circumstances surrounding the deaths of Alphonsine Djiako Leuga, 47, and Loraine Choulla, 18, who were found deceased in their Radford home on May 21, 2024. The review, a Child Safeguarding Practice Review, will examine the involvement of all agencies involved with the case.
Nottingham City Council has provided further details concerning an independent review into the events preceding the deaths of a mother and daughter discovered deceased in their Radford residence. Alphonsine Djiako Leuga, 47, and Loraine Choulla, 18, were located by police officers inside their Nottingham City Council -owned property on Hartley Road, Radford , following a welfare check on May 21, 2024.
An inquest into the deaths of the two women commenced on Friday, January 24th, with Alphonsine's provisional cause of death recorded as pneumonia of uncertain origin. The cause of Loraine's death remains undetermined. The coroner designated the date of death for both women as May 21st, the day their bodies were found. However, Nottinghamshire Police stated that the bodies had been undiscovered for a considerable period prior to their discovery, adding that the deaths were not suspected to be criminal in nature. Nottingham City Council confirmed that a comprehensive review would be undertaken to examine the involvement of all relevant agencies in the circumstances leading up to the deaths. The authority subsequently issued an update on the review following the opening of the inquest. Steve Edwards, the Independent Scrutineer for Nottingham City Safeguarding Children Partnership, remarked: 'The Nottingham City Safeguarding Children Partnership extends its deepest condolences to the Leuga and Choulla family. Adhering to national guidelines, the safeguarding partnership has commissioned an independent 'Child Safeguarding Practice Review' to investigate the tragic circumstances surrounding this case. 'The review will encompass all agencies involved in the case. The Independent Child Safeguarding Practice Review will be made public and, in conjunction with any findings from the inquest, will serve to inform practices across the partnership. 'The Safeguarding Partnership must await the culmination of the coroner’s inquest and the completion of the independent Child Safeguarding Practice Review before issuing any further statements.' The inquest is scheduled to reconvene for a final hearing on a date yet to be determined.
