One tender was awarded by verbal contract — breaching procurement rules and leaving officials unable to account for how much taxpayer money was spent on the contract over a year, the Auditor-General's report finds.

"Testing of procurement documentation relevant to the planning lifecycle found a number of required forms were unable to be produced for audit scrutiny or were incomplete," she wrote. Ms Crisp found that had not been approved by authorised staff and"was not compliant with any of the procurement rules".
The department also did not keep records of all complaints about contract-related grievances, investigation findings or complaint resolutions, the Auditor-General found.In a departmental response attached to the Auditor-General's report, the department admitted it had failed to"systematically" follow procurement rules and stated that a"consultant" has since been brought in to fix the serious compliance problems.
