A Perth childcare centre has been slapped with a record penalty after an unsupervised three-year-old was found unconscious with a skipping rope around their neck.
"It's about as close as you can get to the worst-case scenario without it actually being a child's death," Angelo Barbaro from the Department of Communities said.

The company that owns the centre, G8 Education, has now been handed the biggest penalty in Australian history over the incident, which is two fines totalling $90,000.
