A police investigation is under way in Lancashire after the death of a dementia care home resident following an assault in the facility they lived in.

For example, a prescription for insulin was not recorded and dose directions were only found on the insulin pen, leaving a risk of the right dose not being administered as the label deteriorated on the pen.
The fire evacuation point has recently been repositioned, but signage was yet to be put in place and other procedures had not been updated for the change. There was also an issue with the staff at the home, as the inspectors found a risk for people who showed their emotions physically, if they could not be understood.
A spokesperson for L&M Healthcare, who own and operate Hulton House, said: "We have been working very closely with the CQC and commissioners to offer assurances of continual improvement and progress since the CQC visited. The home openly welcomes monitoring visits and the support offered from the commissioners to assist with continual progress to enable the residents in our care to live full and meaningful lives.
