A review into the Prevent scheme found that Axel Rudakubana, the teenager behind the Southport stabbing attack, should have been referred to the next stage, Channel, after several concerning incidents. Despite being referred to Prevent three times, including once by a teacher who noted his interest in school shootings and violent content, Rudakubana was not escalated, ultimately leading to the tragic attack.

The teenager behind the horror Southport stabbing attack, Axel Rudakubana , was first referred to an anti-terrorism programme by his teacher in Ormskirk.

Today , security minister Dan Jarvis conducted a speech in the House of Commons over the review into Prevent. The ECHO reports it found a high level of compliance by officers with process timescales, assessment completion and adherence to policy that was in place at the time. A month later on November, 15, 2019, the teenager was found to be researching school shooting in America during ICT lessons. On December 3 of the same year, Rudakubana was in an art lesson when he queried why he was able to draw pictures of guns, but not research them on the internet. He then asked: "can we have a picture of a severed head then?"
