'We have identified a significant number of concerns'
Strangeways has been slammed in an report arising from the death of a prisoner. The Prisons and Probation Ombudsman , which independently investigates deaths in custody, has issued a series of recommendations to HMP Manchester after finding 'significant concerns' - and called for a 'disciplinary investigation' into the actions of segregation unit staff to be launched.

But the report said no one at HMP Manchester acted on that, and didn't act quickly enough when he was found in his cell, with a three-minute delay in radioing a medical emergency code referenced.
"It is important that the governor recognises and seeks to address any underlying cultural or staffing issues at the prison." On the day he was found, July 5, 2021, he pressed his cell bell 'a number of times', but it was 'reset' without prison staff speaking to him. "We are concerned that Mr Hussain's punishment of cellular confinement was excessive for the circumstances of his offence," the report added. "Although he said at his disciplinary hearing that he wanted to speak to the mental health team, no one referred him to them.
