Sam Prince had requested a new case manager but no-one was ever appointed

A teenager felt he was "abandoned" by child mental health services months before he was found dead at home.

During an inquest into Sam's death, which started today at Preston Coroner's Court, Assistant Coroner Richard Taylor described the discussions around discharging Sam from CAMHS taking place directly after he requested a new case manager as an "incredible coincidence". Mum Julie added: "He didn't get any help, he was crying out for help, but he was completely left. We felt like you were saying we've tried everything, there's nothing more we can do."

Mum Julie said to Ms Crowther-Jones: "We as a family, and Sam, never requested discharge. The first time it was mentioned was when Sam had requested a change in case manager because he was struggling. Sam's step-dad John said: "We felt like there was a pattern of behaviour. He would experiment and then he would get a sense of guilt and shame around that.
