The family of a devoted dad and grandad from Clayton-le-Moors have called for justice after a catalogue of errors and failings leading up to his death.
David's wife April raced to the hospital and spent 10 hours by his bedside but said "he never opened his eyes again". A CT scan revealed David had suffered an acute subdural haematoma, a bleed on the brain, and he continued to deteriorate until he died on December 27 at the age of 77. Since taking the reins at Mapleford, and following David's death, Mrs Reedy has implemented a series of improvements. "The whole governance, car planning and paperwork has been changed to meet the needs of the people living in the home," she added. "Some staff have left and some new staff have come along."
However, two other medical professionals offered different causes of death, which led Ms Bisset to adjourn the inquest to allow her to obtain further expert evidence. She will also request evidence from the Blackburn with Darwen and East Lancashire Clinical Commissioning Group responsible for the acute visiting service.
