The Scottish Public Service Ombudsman (SPSO) has found that NHS Lanarkshire failed to warn a patient of the risks associated with blood thinning medication, leading to their death. The ombudsman ruled that the health board must apologize to the patient's partner and failed to inform the family about the medication's role in the death.

Staff then administered medication to reverse the effects of the blood thinners.
Following advice from a consultant in acute medicine, the regulator upheld the complaint, writing: "We found that the board failed to warn A of the risks of the anticoagulation medication before commencing the treatment.
