A mother whose newborn baby died a week after being born has bravely shared her five-year battle for answers and the devastating impact it's had on her life. Ida Lock's parents, Sarah and Ryan, have fought tirelessly for justice after concerns regarding Ida's birth and subsequent death led to an inquest.

A mother whose newborn baby died a week after being born has revealed the 'most heart-breaking thing' about her five-year fight for answers. Ida Lock was born to parents Sarah Robinson and Ryan Lock at the Royal Lancaster Infirmary on November 8, 2019. Delays in delivering Ida, when her heart rate decreased, and several 'failings' led to her having to be resuscitated, and she died seven days later after being transferred to the neonatal intensive care unit at the Royal Preston Hospital.

In the months that followed Sarah and Ryan repeatedly asked bosses at University Hospitals of Morecambe Bay NHS Foundation Trust for a 'de-brief' and it was only when they sought advice from medical negligence solicitors that Lancashire's Senior Coroner Dr James Adeley was informed about the circumstances of Ida's birth and death. An inquest, which opened today (Monday February 10) at Preston's County Hall, heard that the trust carried out a Root Cause Analysis (RCA) into Ida's death, but her parents were not involved in the investigation or given a copy of the report when it was published in January 2020. At the inquest Sarah, from Morecambe, described how she and Ryan had managed to get hold of a copy of the RCA ahead of a meeting with trust bosses seven months after Ida died. Sarah had emailed the trust the month earlier, in May 2020, to ask if the report was complete but 'received no response'. 'It was a convoluted way of getting the RCA report,' she said. 'Ryan's sister is a nurse in Blackburn and she knew the bereavement nurse there who knew the bereavement nurse at Lancaster. She eventually got it for us so we could see it ready for the meeting the week after.' The Health Services Safety Investigation Body (HSIB) had already looked into the care provided to Ida and Sarah during the birth. Their report highlighted several 'failings' and 'care delivery issues' but when the couple got hold of the trust's RCA it was like 'night and day' in comparison. 'It said there were no care delivery issues but the HSIB report clearly said there were,' Sarah added. 'The trust's report said that teamwork and documentation were 'outstanding'.' At their meeting in June 2020, Sarah said that head of midwifery Carol Carlisle and obstetrics consultant Mark Davies both 'agreed with' the couple's concerns, but when the trust was asked to provide a 'position statement' in June 2021, this appeared to have 'distanced itself from admissions made,' the inquest heard. 'It was almost like we were back to square one,' Sarah added. Sarah was then asked to describe the impact of having to fight for answers throughout the five years since Ida's birth. 'This is the bit that breaks my heart the most,' she replied, as she began to cry. 'Five years of fighting and that time taken away from us.' Sarah described how she has experienced 'a rollercoaster of emotions' and added: 'Myself and Ryan want to be sure that lessons are learned and effective changes are made so that history doesn't have to repeat itself. I hope that no parent has to go through what we've gone through.' The inquest had heard earlier how Sarah had felt she was being 'accused' and blamed for her daughter's death after a doctor told her that her placenta looked 'gritty and fatty' as if she had smoked during her pregnancy. Sarah confirmed she had 'never smoked' in her life. University Hospitals of Morecambe Bay NHS Foundation Trust, which is being legally represented during the inquest along with the midwives and the Care Quality Commission, has admitted there had been 'care delivery issues' during Ida's birth. The inquest, which is due to last 17 days, continues
