A 41-year-old man from Morocco was diagnosed with a rare form of tuberculosis, specifically genitourinary tuberculosis, after experiencing severe swelling in his left testicle. The swelling, which led to the release of a thick, milky fluid, prompted medical intervention. Doctors suspected TB despite the absence of typical symptoms, suggesting the infection originated outside the lungs.

A 41-year-old man from Morocco was diagnosed with a rare form of tuberculosis (TB), specifically genitourinary tuberculosis ( GUTB ), after experiencing a significant swelling in his left testicle for eight weeks. The swelling, which reached 15cm in size, began leaking a thick, milky fluid three weeks prior to his medical consultation. Doctors suspected TB as the cause, despite the patient reporting no typical symptoms such as fever, cough, night sweats, or weight loss.

This suggested the infection might have originated outside the lungs, a less common scenario.The patient denied exposure to TB in the past and had no history of urinary symptoms or scrotal injury. A physical examination revealed a firm, irregular, and non-tender left testicle, while the right testicle appeared normal. Ultrasound and CT scans confirmed the scrotal wall thickening and ruled out bladder and kidney involvement. Genitourinary tuberculosis represents about 20% of extrapulmonary TB cases, where the infection affects body parts other than the lungs. It's even rarer for TB to infect the testicles, occurring in only 2 to 4% of GUTB cases.Given the potential for the swelling to become cancerous or spread, surgeons opted to remove the affected testicle. During the procedure, the testicle was found to be significantly enlarged with a necrotic and heterogeneous appearance. The patient recovered well from the surgery with no significant side effects reported. TB, a potentially serious bacterial infection, is treatable with a six-month course of antibiotics. Early diagnosis and complete adherence to the prescribed medication are crucial for successful treatment and to prevent the spread of the infection
