While most people picture a hospice as a building, around four out of every five Severn Hospice patients receive care in their own homes. The nursing and social care is provided by the charity's Hospice at Home team, which covers the entire county of Shropshire, as well as parts of mid Wales. They strive to deliver the highest quality of support, working alongside the county's GPs and other health and support workers.
The team provides day and night care and also runs the charity's 'virtual hospice', offering ward-equivalent care in patients' homes. The Hospice at Home service typically supports patients in the last six weeks of their illness, allowing them to die at home, free from pain, surrounded by loved ones. Sarah Hill, a Hospice at Home nurse practitioner, emphasizes that the service is for people who wish to receive care or die at home during their final weeks. Lynn Davies, a virtual hospice practitioner with 20 years of experience at Severn Hospice, highlights the team's commitment to finding the best ways to help people remain at home, understanding the anxieties some families may have about inviting people into their homes, especially during a difficult time. Davies explains that the team works closely with families to ensure the care provided is comfortable and minimizes disruption. Michelle Knott, a care co-ordinator for the team, stresses their awareness of the challenges patients face and the benefits of providing care in a familiar and comfortable environment
