Regan Tierney was stabbed to death in a 'murder-suicide' by her ex-partner in 2019
There were ‘missed opportunities’ to protect a young mum found stabbed to death at her Salford home after state agencies gave her ‘insufficient’ support, an inquest has concluded.
Regan’s medical cause of death was recorded as 1A multiple stab wounds Over the course of the inquest, the jury heard evidence from a number of witnesses including police officers, health professionals, and Regan’s friends and family. She added that Patten became 'nasty' after drinking, and that this culminated in an incident just before her niece's first birthday, she told the court, when he 'beat' Regan while the couple were at a party. Shannon received a panicked call from the host, who told her Regan was found ‘choking on her own blood’. At the hospital, Shannon found her sister sat in the back of a police van with a broken nose.
In her log read out to the court, PS Bailey recorded the incident as ‘clearly a two-sided argument with empty threats’. The officers later went to see Regan, who PS Bailey said appeared ‘happy’. It took GMP officers three days to visit Regan. Jurors were shown police bodyworn video footage which showed her telling the officers that Patten had been ‘harassing’ her and making threats to ‘ruin her life’ and ‘throw acid in her face’ since they had broken up around seven weeks prior.
However, he accepted the assessment was ‘incomplete’ and, in parts, ‘incorrectly’ filled out, when pressed by Emma-Louise Fenelon, representing Regan’s family.
